Реформування публічного управління охороною здоров’я в Україні: регіональний вимір (на прикладі Ізмаїльської територіальної громади)

Abstract

UK: Стаття присвячена комплексному аналізу механізмів публічного управління розвитком охорони здоров’я на регіональному рівні (на прикладі Ізмаїльської територіальної громади Одеської області) та розробці науково обґрунтованих рекомендацій щодо їх удосконалення в умовах децентралізації. Дослідження проведено із застосуванням системного та компаративного аналізу, ситуаційного аналізу, статистичних методів при вивченні особливостей функціонування системи охорони здоров’я Ізмаїльської територіальної громади. У результаті виявлено ключові проблеми регіонального публічного управління охороною здоров’я в Ізмаїльському районі: кадровий дефіцит, застаріла інфраструктура, фінансова нестабільність медичних закладів. Запропоновано пріоритетні напрями вдосконалення механізмів публічного управління розвитком охорони здоров'я, зокрема: впровадження комплексної системи мотивації медичних працівників, модернізація медичної інфраструктури, забезпечення прозорості фінансових потоків, інтеграція інформаційних систем та активне залучення громадськості до контролю за якістю медичних послуг.

EN: The article is devoted to a comprehensive analysis of public management mechanisms for healthcare development at the regional level (using the example of Izmail territorial community in Odesa region) and the development of scientifically substantiated recommendations for their improvement under decentralization conditions. The research was conducted using systematic and comparative analysis, statistical methods, and situational analysis when studying the peculiarities of the healthcare system functioning in the Izmail territorial community. As a result, key problems of regional public healthcare management in the Izmail district were identified: staffing shortages, especially primary care physicians, outdated infrastructure, financial instability of medical institutions due to imperfections in the National Health Service of Ukraine payment system and high staff turnover. The study defines the organizational features of medical care in the region, including the functioning of the treatment facilities network and the formation of a hospital network within the third stage of medical reform. Priority areas of inter-municipal cooperation between territorial communities are substantiated, which include consolidation of hospitals according to the hospital district development plan, organization of a network of points providing the most demanded outpatient services, and implementation of an inter-district strategy for human resources development. The article proposes priority directions for improving public management mechanisms of healthcare development, specifically: implementation of a comprehensive incentive system for medical workers, modernization of medical infrastructure taking into account the real needs of the population, ensuring transparency of financial flows, integration of information systems to improve management decision efficiency, and active public involvement in monitoring healthcare service quality. The research reveals significant disparity in hospital bed availability in the Izmail district (30.9 per 10,000 population) compared to the regional average (78 beds), highlighting a critical infrastructural gap that requires targeted policy interventions. The findings emphasize the urgent need for developing standardized inter-community cooperation frameworks in healthcare management to ensure equitable access to medical services across the Izmail territorial communities during the ongoing decentralization process.
